The firm mattress myth#

"Firmer is better for your back" is one of the most persistent myths in mattress buying, and the evidence doesn't support it. A systematic review of 39 studies found medium-firm, not firm, best supports comfort, sleep quality and spinal alignment for most people.1 A controlled trial even found a medium-firm mattress reduced back pain and helped people fall asleep faster than a high-firmness one.2 So before you buy firm by default, make sure you actually need it.

Who genuinely benefits from a firm mattress?#

  • Heavier sleepers: more body weight sinks further into any mattress, so a firmer core stops the hips dropping and the spine sagging.
  • Front (stomach) sleepers: a firmer surface keeps the hips from sinking and over-arching the lower back.
  • Some back sleepers: those who feel "swallowed" by a softer bed may prefer the on-top support of a firmer one.
  • People who simply prefer the feel: comfort is individual, and a firmer surface that keeps you well aligned is a valid choice.

Who should avoid a very firm mattress?#

Side sleepers and lighter people usually do worse on firm. Without enough give, the shoulder and hip can't sink in, so the spine bends away from neutral and pressure builds at those joints, the opposite of what you want. If you wake with a sore shoulder or hip on a firm bed, that's the signal to go softer.

How to choose a firm mattress that still sleeps well#

  • Look for a comfort layer, not a board: the best firm mattresses pair a supportive core with a thin pressure-relieving top, firm support without a punishing surface.
  • Consider a hybrid: firm pocket springs give support and airflow, with a comfort layer to take the edge off.
  • Check durability: quality firm foams and springs hold their feel well over time, while cheap firm foam can soften quickly.3
  • Use a home trial: firmness is best judged over weeks, give your body time to adapt or to tell you it's too hard.
