The "firm mattress for back pain" myth#
For decades, the conventional wisdom was that a firm mattress was better for your back. This recommendation originated partly from medical advice in the mid-20th century and was enthusiastically adopted by mattress retailers (firm mattresses are cheaper to make than well-cushioned ones). The research has since told a more nuanced story.
A landmark Spanish study of 313 adults with chronic low-back pain found that participants on medium-firm mattresses reported significantly less back pain and disability than those on firm mattresses. More recent reviews have consistently found that neither firm nor soft is universally better — the right firmness depends on how you sleep.
Firmness and sleep position: the key relationship#
The ideal firmness is the one that keeps your spine in neutral alignment throughout the night. This depends primarily on your sleep position:
Side sleepers — soft to medium#
Side sleeping creates concentrated pressure points at the shoulder and hip. A mattress that's too firm won't allow these points to sink enough, causing lateral spinal curvature. Side sleepers need enough softness to let the shoulder and hip sink to the level of the waist, keeping the spine level. Medium-soft to medium is typically ideal for side sleepers.
Back sleepers — medium to medium-firm#
Back sleepers need support under the lumbar curve and across the entire back. A mattress that's too soft allows the hips to sink too deep, flattening the lumbar curve and creating strain. Too firm creates pressure at the shoulders and heels. Medium to medium-firm typically provides the right balance.
Stomach sleepers — medium-firm to firm#
Stomach sleeping creates the most significant spinal stress — the lumbar spine hyperextends if the hips sink too far into the mattress. Stomach sleepers generally need a firmer surface that keeps the hips level. This is also the most spine-damaging sleep position, and switching to back or side sleeping is medically advisable where possible.
Body weight and firmness#
Your weight affects how any given mattress feels and performs:
- Lighter sleepers (under 9 stone / 57 kg): Compress mattress layers less, so may find medium mattresses feel firm. Tend to do better on soft to medium.
- Average weight (9–14 stone / 57–90 kg): Compress mattresses as the manufacturer intends. Firmness ratings on product pages apply most accurately to this range.
- Heavier sleepers (over 14 stone / 90 kg): Compress mattress layers more, making any given mattress feel softer. Often need to size up in firmness to achieve the desired feel.
